Twenty-two semi-structured qualitative interviews were conducted with concerned significant others (CSOs) about their perceptions and experiences during the toxic drug crisis, with a particular focus on how tough love constructs shaped their relationships with and responses to loved ones at risk of overdose. Understandings of tough love were found to be widely varied and deeply intertwined with decisions around support, boundaries, and compassion.

Decisions were mediated by three main factors:

  1. Perceived potential outcomes of different responses to their loved ones.

  2. Beliefs and values about love, compassion, drug use, and personal agency.

  3. Available personal assets or social capital.

Participants often expressed significant uncertainty and regret, which they attempted to mitigate through the psychosocial concept of “boundaries,” contrasting with dichotomies of tough love and enabling. The findings suggest that decisions about engagement and support involve complex negotiations within the highly pressurised context of a toxic illicit drug supply. Moreover, inconsistently understood normative language shapes and complicates relational negotiations for CSOs, highlighting the need for theoretical frameworks to guide decision-making within both moral and pragmatic limits.
